I was able to find the addresses for Covers Blown and Harming Enemies/Civilians. Here's everything I've got:

On Flatline targets: The 3 mobsters are "civilians/innocents" until rescuing Agents Smith. After the Smith cutscenes, the 3 mobsters become targets. Rescuing Smith after killing the mobsters will not change CivilianKills or TargetKills values, but you can still complete the mission with SA. This shouldn't ever be an issue though since it's always(?) optimal to rescue Smith first thing.
Guards counted under GuardKills1: DOAS, VY, FL(Bodyguards only), MOTC(Birds only), YBWO, DOTM, TDDUP, AHOC(Bodyguards only), ADWTD(Masked Guards only)
Guards counted under GuardKills2: CD, FL(Security only), NL, MOTC(Security only), ADWTD(Security only), AXXV
Gator Gang value does NOT reset to 0 after mission completion, only resets when restarting DOTM.
When I search for Witnesses, I notice the only value I can find the the MAX number of witnesses that were in the mission. If I killed them, the value did not go down, so there may be a witness death address as well, even though I couldn't locate it... Witness address still eludes me. It's a dynamic address that I can find every time, but the address resets every mission. Not sure how to figure that one out.
I also want to throw this out there: I used this guide to try to find the Witness address every time with 'pointers', but I could never get it working properly.
